  • SKILLED NURSING

    Hospice Nurses


    The Hospice nurse completes a comprehensive assessment that provides “family-centered care” at the client’s home which provides comfort while working to alleviate the stains associated with a terminal illness.  The goal of a hospice nurse is not to offer complete recovery but to ensure that the patient dies peacefully and with integrity. 


    Services include but are not:

    • Pain & symptom control 
    • Calibration with physicians and other professionals to plan care 
    • Make frequent visits to patient to evaluate their condition 
    • Provided support and comfort customized to individual client/family needs
    • Supervision of medication administration 
    • Manage sudden symptoms of the diseases end phase
    • Coordinates a plan of care with the interdisciplinary (Chaplin, SW, Medical Director, CNA, Volunteer) to provide support to client/family

  • HOSPICE AIDE

    Hospice Aide

    Medical Social Workers 


    We understand your family is experiencing a high level of pressure and stress at the present time. Our social worker can help you and your family cope with emotional, physical, and financial stress. With the help of our social worker, you can acquire help and support from various community and local health resources. Our social worker's goal is to be an advocate for each patients end of life wishes.

    Chaplain


    Our chaplain will provide non-denominational and non-judgmental support and assistance. Upon request, we can connect you with various spiritual/faith groups. Our chaplain will be there for you if you want spiritual guidance, counseling, or a sympathetic listener. Our chaplain offers prayer, companionship, and other services you need at this crucial time.

    Optimal Hospice Care provides bereavement support to our client’s family, up to 13 months after death.

Our volunteers have compassionate hearts that help our clients live a meaningful and comfortable life. They look after the safety and well-being of your loved one through sensitive and warm delivery of activities/services. Our trained volunteers serve as a supporters and hospice care providers of your loved one.
